Eight small bones form the tunnel, with the palmar surface composed of soft tissues and the transverse carpal ligament.

The median nerve passes through this tunnel, along with the flexor tendons. Compression of the nerve or tendons will result in pain.

  • How common is carpal tunnel syndrome?

    • Carpal tunnel syndrome affects 4-10 million Americans, according to studies. Middle-aged to older people are more likely to be affected by CTS, and females are three times more frequently affected than males.

  • What causes CTS?

    • When the median nerve and flexor tendons become compressed by swollen tissues, this blocks nerve impulses and slows function and feeling in the hand. With chronic nerve and tendon compression, the patient suffers permanent damage.

      Certain conditions increase the likelihood of CTS, such as pregnancy, obesity, thyroid disease, arthritis, diabetes, and injury to the wrist. In addition, CTS is associated with overuse of the wrist and hand.

  • What are the signs and symptoms of carpal tunnel syndrome?

    • CTS usually affects the dominant hand, but it can affect both. Symptoms include numbness of the fingers and thumb, as well as burning, tingling, and/or squeezing of the fingers and forearm.

      Symptoms are often worse in the morning upon awakening, or can occur in the middle of the night. The weakness associated with CTS can affect holding a cup or book, twisting a lid, grasping an object, or bending the wrist.

  • How is CTS diagnosed?

    • The diagnosis of carpal tunnel syndrome is made based on a description of the painful symptoms and a thorough physical examination. Certain tests are used to identify weakness of the muscles supplied by the median nerve, and the doctor will check for decreased sensation in the hand and fingers. Diagnostic tests used include:
      . Nerve conduction test - This involves placing electrodes on the elbow side of the wrist region. A mild electrical current is used to stimulate the nerve to assess damage. With CTS, the impulse will take longer to get to the hand. In addition, the degree of muscle function is assessed by placing a small needle into various muscles supplied by the median nerve.

      . Imaging tests - X-rays and MRI scans are often used to rule out other problems and assess the bones and soft tissues of the wrist and hand.

  • What is the treatment for carpal tunnel syndrome?

    • Medications - There are several drugs used to treat the symptoms associated with CTS. Nonsteroidal anti-inflammatory drugs (NSAIDs) are used to decrease swelling related to inflammation. For severe pain, opioid analgesics can be prescribed short-term.

      Splinting - To alleviate pain and keep the wrist in alignment at night, a wrist splint is used. In addition, splinting decreases the pressure on the median nerve.

      Corticosteroid injection - The doctor can inject a steroid agent into the carpal tunnel to decrease inflammation and offer long-term pain relief. In a clinical study involving CTS treated with steroid injection, the success rate was 77%, and 63% of patients reported pain relief that lasted for six months.

      Carpal tunnel release surgery - When symptoms persist and are severe, the doctor can perform either open or endoscopic surgery to release (sever) the carpal ligament and separate it from the median nerve.
